Worksite Health Disease Management
Provides a variety of health and wellness services to employees
Club Health Worksite Health Management combines onsite health evaluations and comprehensive blood draws with post-screening follow up to keep healthy people healthy and to identify individuals with specific health management needs. The goal of this program to identify and manage members before they become a claim.

- The onsite services include a thorough 36-test panel, blood pressure reading and health questionnaire.
- Following the screening, employees have access to a secure website where they can:
- review lab results on-line within 48 hours
- get general health information
- use health tools
- Employees also receive a post-screening personal health report via mail within 7 business days, which includes:
- overview of results
- instructions for using the secure website
- detailed lab results
- Interactive Health Index (employees can see how they compare to national guidelines)
- risk analyses (general health, coronary, diabetes, PSA, weight, behavioral health)
- trend analysis (compares past and present screenings)
- health care calendar
- Employees also have access to a series of telephonic HealthFocus courses, which consist of 6-8 sessions lasting 15-30 minutes each. Topics include:

- Lifestyles for successful weight loss
- Managing cholesterol levels
- Managing and preventing high blood pressure
- Better nutrition
- Controlling your asthma

- Clients receive an aggregate report detailing:
