Population Health Management

Population Health Management supports members in all stages of health. Backed by clinically proven behavioral assessment and modification tools, the program offers personalized coaching to empower participants to set goals that are incremental and achievable. Enhanced online tools and education provide participants the best opportunity for achieving health goals. Wellness
Targeting healthy individuals—employees and spouses—along with those at risk for developing chronic conditions, Wellness empowers participants to develop personalized goals for managing stress, improving nutrition, quitting smoking, and modifying other lifestyle behaviors that impact health. Wellness Coaching utilizes member-centric motivational interviewing, cognitive skill building, and behavior modification techniques to help individuals identify life goals, resolve ambivalence, and build conviction for change. Other program options include a Wellness Portal featuring a health assessment, symptom checker, health info center, 24/7 live nurse chat and more; at-home and onsite biometric testing; and onsite health seminars that address common health challenges.
Disease Management
American Health’s URAC-accredited Disease Management targets eight prevalent conditions for which evidence-based guidelines are established to impact health and measure improvements: Asthma, Diabetes, Chronic Pain (from osteoarthritis, rheumatoid arthritis or low back pain), CAD, CHF, COPD, Hyperlipidemia and Hypertension. The goals of American Health’s Disease Management include improving participant activation, in other words, increasing an individual’s skills, confidence and ability to manage a chronic condition(s) as determined by the Patient Activation Measure™ (PAM™); and effecting improvements in the health status of participants by providing education, tools, and support that promote good self-care practices and adherence to evidence-based care guidelines. Utilized by leading health care organizations, the PAM is a clinically-proven behavioral evaluation tool that assesses an individual's confidence, knowledge, skills, and ability to self-manage a chronic condition. Results of the assessment are used to help coaches collaborate with participants to set behavior-change goals and pursue action steps that are tailored to the needs of each participant.
