Description

• Telephonically educate members on self care focused behaior modification and member empowerment.
• Assist members to manage chronic disease through education and coaching.
• Refer members to appropriate resources.
• Conduct sound nursing assessments and offer alternatives based on established nursing guidelines.  

Requirements

• Telephonic disease management or patient education preferred.
• Minimum 3 years of clinical experience.
• Excellent written, verbal and interpersonal communication skills.
• Basic counseling skills and knowledge of basic community services.
• Familiar with DM protocols and outcomes.
• Positive, enthusiastic, helpful personality.
• Organized, able to set priorities and work effectively in a limited space with minimum supervision, but also work well as a team member.
• Ability to work evening hours.
