Telephonic Nurse Health Coach (RN) (Part-Time --- Worthington, OH)
Description
- To increase member participation and retain customers
- Apply disease management interventions to identify member population on a daily basis
- Conduct initial & ongoing assessments and member action plans for engaged participants
- Serve as a key contact/expert for enrolled members, their significant others and physicians for:
- Telephonic health coaching and education
- Re-stratification after a new medical event
- Medication management
- Community resources
- Responsible for day to day documentation into the disease management system of all interactions with members and physicians
- Perform day-to-day health coaching for selected high risk participants
- Establish available support systems and be versed in methods of researching and interviewing community resources that will assist the member in achieving maximum psychological, social and physical health
- Identify cases that would benefit from alternative care through assessment and evaluation of the member needs and available resources
- Understand disease management concepts such as roles, philosophies, principals, liability and confidentiality issues and apply these concepts in developing member driven action plans and goals based on the needs of the participant
- Be knowledgeable in applying problem solving techniques to the disease management process, will analyze and evaluate its effectiveness and implement alternatives when appropriate
- Maintain a professional status within the community through participation and continuing education
- Participate in American Health’s committees and will perform all assignments within the professional scope of the organization
- Comply with policies and procedures and conforms to the URAC standards while performing job functions
- Other duties as assigned by management
Requirements
- Active, unrestricted RN licensure in State of Residence
- At least three years experience in an acute hospital setting or a minimum of five years experience in a specialty field
- Diverse medical background with exposure to multiple environments
- Excellent verbal and written communication skills
- Self-motivated
- Efficient time management skills
- Proficient in use of Windows and Word
- Experience in wellness programs preferred
- Ability to work evening hours
