Case Manager, RN (Full-Time -- Buffalo, NY; Telecommuters; Worthington, OH)


Our case managers provide case management telephonically and their job duties include:

• Assess patient’s needs through evaluation of social and medical history
• Develop a plan of care with the providers and patient
• Continually evaluate the plan of care based on changing needs
• Monitor the quality of care
• Act as a patient advocate
• Identify cases that would benefit from alternative levels of care and resources

Requirements:
• Active, unrestricted Nursing License
• Certification as a CM preferred (CCM)
• Ability to utilize web-based software effectively
• At least 3 years in an acute hospital setting or a minimum of 5 years in a specialty

This position is based out of Amherst, NY but is also eligible for telecommuting.
